EAD and AP renewal- How to?

Hello Gurus,
I am planning to renew EAd and AP by myself. what are the forms other than EAD and AP forms + checks need to be submitted? Any other documents like DL or PP required?

For EAD: fill up form I-765, BCIS fees check $120, 2 photographs copy of I-485 receipt (if you have approved I-140 also send the copy)
For AP: fill up the form I-131, BCIS fees check $110,2 photographs copy of I-485 receipt

If your service center is NSC send the color copies of DL and first page of Passport that has your photo, DOB, Name
