DV2009 need help on NL package instruction

Hi all,

I am a winner of DV2009 with CN 2009AS12xxx, just got my NL last week. Can someone verify what is included inside the NL package envelope, especially on the attached instruction? This is what I got for the attached instructions:

1. 1 page of "Letter to DV-2009 Selectees"
2. 2 pages of "Instructions for DV-2009 Selectees"
3. 2 pages of "General Photo Requirements" (both pages are identical)
4. 1 page of "Adjustment of Status Instructions" (I miss something in here)
5. 1 page of Processing Stages

My real question is: Can someone verify how many pages for the "Adjustment of Status Instructions"? I think I only got the second page for that, the one with the "Adjustment of Status Fee Payment" title.

If I really miss one or more pages, can someone help to scan that page for me or give me the links where I can find it on the Internet? I tried to Google but no luck. Your help is really appreciated. Thank you so much in advance.

I have two, the first with the title, and the second with a form for the DV fee. Note that on the very bottom of each page you can find the name of the section this page belongs to, e. g. each of the two pages about adjustment has "adjustment of status instructions" on the bottom part.

I am from Indonesia, living in US right now on F-1 visa, will graduate on Aug 08 and plan to take my OPT after that. AOS is the only choice for me since there is a risk for me to take CP. Can you help me with the document? Thanks in advance.
