DV2006 DS230 under 16s - who signs ?

Who signs the DS-230 form for DV2006 application - I have a 12yr old - is she allowed to sign it - or should I the parent sign this form.

for a child under 14 years - a parent must sign and next to the signature he/she needs to indicate the relationship to the child (mother, father).
