Dv-I-555 green card form

Hi all,
I was wondering if anyone has had this problem before.
I entered the U.S in 7, 2005, feb on Dv immigrant Visa, we had our I-555 stamp stamped in Los Angeles. They took our finger prints & we sign on a form it was a I-555 green card information ( something like that) form. But when they returned the paspport they handed me my ( the principal applicant's I-555 card information form back to me with a photo attached ). :( We recieved our SSN card in about a week and then I had to leave the State as I was working in Australia. As I had a 1 yrs contract with the employer in Australia.which ends in july 2005)
I plan to return to U.S permanently after 4-5 mnths. Questions:
1.Can I travel back to the U.S with the I-555 stamp?
2.The form they return to me now I doubt was a information gathering form for the actual plastic card, , it is with me what do I do with it, do I have to go to the U.S with it, I was planning to go back 4-5 mnths later.
3. Do I need to apply for permit to go back to the U.S and will it effect my permanent residency, as I came back without the actual plastic card.
anyone's help will be greatly appreciated

Yes.

You weren't supposed to receive anything from the immigration officer, but your pasport with I-551 annotation. I guess you can turn it in next time you are coming over. Or get your wife in the states to turn it in.

??? what? you don't have an actual plastic card, do you?

If you are going to stay outside the US for more than a year, you have to apply for a reentry permit WHILE you are in the US. By the way, if you are working in a different country, you are forfeiting your GC.

thanks for the reply sorry I made a mistake I came back without the plastic card. And I am only working in Australia for another 4mnths as I have to get my super money which is a lot even in U.S dollars and I was working in Aus even before I got the actual lottery. Does it still effect my residency?

do you or do you not have a plastic card? If you don't, then you should and can use the stamp to come back. If you do, then you should use the plastic card to reenter.

If you work in a different country, you are showing intent to abandon your residency in the States.
