DV 2009 Forms Help Please

How are you doing?
I'm an F1 Student who applied for DV lottery 2009 and got the notification letter.Then,I received the second letter from Kentucky saying that they finish processing my case and i need to contact local USCIS to adjust my status.
That was great news. But, then i schedule an info pass with the local USCIS.
I showed the 2 letters that i received from kentucky to the clerck ( i guess since she was at the other end of the window). She told me that i need to fill out the following forms:
i-485,I-765,I-130,I-131(extra),G325,I-864 and i had to pay 1010 . That seems fine . But , when i read through forums, i soon release that first i need to fill out G325-A instead of G 325 and second i don't need to fill out I-130 since i'm in the DV lottery section. Now, i 'm doubting about I-864 forms . Should i fill this form or should i fill another form.
That was a little bit frustrating. Is there any link that provide the required forms needed for an F1 to adjust status through DV lottery.
Also, i've been reading forums for the past 2 weeks and previous experience provide help but the procedure has changed. For example, now you can't pay at the cashier instead you have to mail the forms ( package) and wait for the receipt.
In my understanding, for DV 2009 , the applicant should fill
-1--I-485 form
-2--I-765 form
-3-then, wait for his number to be current ( october 1)
-4-Send the package to Chicago
-5-Get the receipt from Chicago office through mail
-6-Submit the receipt and 375 to Department of state
-7-Get the receipt from the Department of state
-8-Do the medical exam
-9-Do the interview

I'm here confused too, I didn't even send any form from the NL yet. DId you send all those back?

When I went to USCIS (infopass too), they said nothing and threw me a "AOS package" with I485, G325A, I765 and I131. No more. You got more forms than me... I know that I have to send these on/after October 1st + I693.

DId you submit the Department of State fee?

For the medical try to get your immunization record, if you have none, get a lab test to prove which vaccines you have so you have to pay less.

I'm lost here, USCIS don't help much...

No, i didn't send any forms to the NL yet. I'm trying to assemble the package "AOS package" . When i went to the Infopass, she pulled up a paper and wrote this list of forms. When i checked this forms online , some of them like I864 don't even apply for our cases (DV) .
Did you actually get a package or did the clerck give a piece of paper.
In case you got the package, does it contain I485,G325A,I765 and II131 only or does it have othe forms.
Regarding the department of state fee, on the second letter it said that we need to get approved for adjustment of status , get the receipt and email them the receipt with their fees. Am i correct? So,i think that we should get the receipt first after submitting the forms and then pay the Fees for Department of state. Regarding the date about submitting the forms, mine too said that it will be applicable on/after OCter 1st.
About the immunization record, can you please tell me how and where to get it?

I went to USCIS. just to get more info about this DV issue but nobody knows anything. The receptionist in a rude way gave me the "AOS package" which includes all those forms plus the I693. If you go again to USCIS asked for the AOS package, I didn't ask but they gave me this but no help in when, where, and if I qualify to do AOS.

About medical, you can go to your doctor and ask for a lab exam to determine which imminizations you have. Since I didn't have any records, I did the lab exam, and I got the immunizations that I didn't have. The medical is simple.

I don't know what to do, if AOS or CP... some says that I may be running of out time because of the fbi check and will miss GC chances.

But I have been living in the US for over 5 years, if I do CP, do I still need police certificate? I don't want to run of time because of something that I can't do anything to speed up the process (seems unfair).

It is frustrating to go to ask for info and nobody know about DV even though it is does occur every year at the same period for many previous years, isn' it?
Regarding AOS or CP, you should check with the Bulltein and see if your number is current. Last time i checked, ( yesterday), all countinents ( asia, europe..) are current except Africa. The rank number in Africa is currently 47000. So go and check your number.If your number is still high, compare last 2 months( progress) and you will have a clerarer vision if your number will be processed from now until December.
If you do CP which i don't recommand, you will need police report.But i 've been reading some forums and AOS don't need to do police report since FB will check it.
I'm sorry to keep bugging about the medical, does the doctor have to fill a INS Form after my visit.

Okey, i finally got it. Form I693 is the medical report that i need to do and check with local doctors about it. Sometimes simple things can get complicate especially if the receptionist does not provide help.

Choosing between CP and AOS depend on you. IF you have a higher number and check the bultein and estimate that your number have less than 50% of getting current, then you do CP. But, in case your number is lower , AOS will be easier , faster and will save all the headach from booking a flight...
So, really check your number and make the decision.
