Duplicate H-1 Approval

I lost my original H-1 Approval copy and would like to apply for a duplicate. Can anybody provide me with the forms and fees for this process.

Recently I applied for a duplicate copy of my H1b approval using the I-824 form. I paid $120 but now the fee has increased to $140. Look in the INS site for the form and instructions.
