DS230 questions

Hi Raj,
I am thinking to write UNKOWN for Question 16 and Question 18 ( date of birth of mother and father)
The reason is my father is 62 years old and he does not know their parents DOB. Would anyonw question during the interview if he writes UNKNOWN???} or do you think gussing would be the best. The only reason i worry that what if they ask for the proof

Also, my father going to stay with me when he comes to US but the thing is we are thinking to sell the house and may be it will be sold before he gets the visa and now i am confused what should I write for Question 10 (parmenet address in US where you intend to live)
Can I write UNKNOW? because the form says 'If known'

{By writing 'unknown' in this case he is not mis-representing
anything. So, it should be OK}

{The address where you presently stay is the address you
can provide, unless you have an idea where you would
move to. So enter this address as the permanent address
for now. This can be changed later on either during the
interview itselft or when they enter US.

Another option is giving the address of a relative or friend
as your permanent address }

Format for DS230

Couldn’t find any where instructions to fill DS230 and hence I’m asking a couple of simple questions for filling in DS-230:

1) Is there any guideline saying whether the DS230 has to be in capital letters or will mixed-case suffice?

2) Does it need to be printed back-to-back like the NIV app?

3) For #25 -- previous visits to the US or to residence in the US, can one be generic and say for Location "Washington DC metro area" if one has lived in multiple cities in the same geographic area?

{There is no instruction specifically for this question. Either
is OK, but you would know it is very difficult to read when all
the matter is written in caps. So, fill it in small case with caps.

{There is no specific requirement like that. But it looks good
printed back-to-back. That is how I had done}

{What you need to fill under 'location' is just the city and
state. So fill in the actual information if you can}

Name Issue - PISKA

Piska,

I ran into the same issue. Just had a meeting with my Lawyer on late friday (3/26) evening. I have 6 names including my lastname. There's a total of 8 combinations of names used on various official documents. (all in good faith, usually a screw-up by government agenicies)

My Lawyer sent an e-mail to Chennai consulate on late Friday evening. 'll post the reply from the consulate once I hear something back.

In the meantime, I have signed an affidavit (notarized by my Lawyer) that all these names are my aliases.

How did you get around with the name situation? Did you have to provide any special documentation to the consulate? (or) the whole thing wasn't even an issue at the consulate?

In my case, the name mess up was in my passport issued in india which I noticed only 3 months prior to my cp interview. To get the name changed in the passport, I had to place a name change advt in india and in usa and wait for 30 days and then apply for a new passport. SO in my case, I got it rectified before my cp interview just to avoid complications (if any). I dont think it would coz any issues had i not done the name change bcoz all other academics documents and usa non-immigrant visa papers had correct name!

All i can suggest is put your name as you want it to appear on your physical GC. And carry appropriate BC, school leaving certificate and affidavits of birth to strenghten your claim around your name.
