DS156/157 Questions (electronic app/Q-29)

Hi,

I am trying to fill up the electronic version of DS156 and have a question regarding Q29.

29. Have You Ever Been in The U.S.? Yes No

If yes I have to give the date visited and duration of visit. I am currently in US applying for revalidation so what should be the answer to this ? Are we allowed to attach sheets with extra info with the electronic forms ?

I have read all the posts in this forum and did not find an answer to my question. Please believe me when I say that I have put up questions which have been unanswered previously.
It will be helpful to atleast know if paper attachments are allowed with electronic DS156 forms.

Answer YES and enter the date of your first entry -to- "today". If you can't enter the word "today" put today's date.
