DS-230 Part 2, Item 29 - for dependent wife

On DS-230 Part 2 for dependent wife, what should I specify for
Item 29 (Name and Address of Petitioner)? My name or my Employer\'s name?

it will be your name and address. i am assuming that ..

she is on H4 and you are primary applicant.

You employer\'s name

I checked with my attorney and he says write your employer\'s name. H1 , H4 are all sponsored by your company. So write your employer\'s name and in paranthesis write (Derivative Applicant)

Should be Employer\'s Name. It is employment based.
