DS-2001 Help please urgent

Hi CPgurus,


I don't have any form called DS-2001 in the packet I received from NVC. IS that normal and to what address I will get notified once I get interview date from Consulate?

You and your attorney, both will be notified of the interview date, along with P4.
 
Thanks Alren. Just wanted to know id DS-2001 is part of the forms sent out by NVC after fee bill.
 
Yes.I filled DS-2001 when i send my packet 3.In DS-2001 you can tell the consulate to send the packet 4 to that mentioned address.

In the end of DS-2001, it asks you to check

I intend to apply
1. Alone
2. Together with my spouse (Print first name)
3. Together with my spouse and the following minor children:
(Print first names of each child who will accompany you)

If the spouse (no kids) plans to go for interview later as follow-to-join, Which one should they check, 1 or 2? Both DS-2001s will be sent to the NVC at the same time with their DS-230s. Thanks.

My I-140 has my old address and I have updated the new address in the ds-230 part 1. Where will I receive my packet 4 ? Is it to the address mentioned in DS-230 part 1? or Should I call NVC to get it changed?

What is DS-2001? I did not receive one, so obviously I did not send it out along with my DS-230 PartI and also I did not find it on http://travel.state.gov/ web site. Can someone please enlighten me? Thanks.
