DS-157 Question???

Hi Gurus,
I'm going to Vancouver for H1B revalidation Next Week.

I've few questions about Form DS-157
1) From which website do we have to download Form DS-157???, when we go through our appointment confirmation letter link through NVARS website it downloads the one which expires 07/31/2005 (I think created on 07-2002) . While if we go through AMCITS website and download DS-157 it opens up the new one with expiry date as 09/30/2008 (with created on date as 04/2006)

2) Do we have to fill the Form in PDF itself and print it or download it and then fill it up with the pen. It looks like impossible to fill it online due to space limitation and new line spacing problems etc.
Is it mandatory to Fill this Form in PDF and print rather then write with PEN.

3) For the Questions in Form like 4 and 13 which doesn't apply to me, should I leave it blank or write "none".

4) For Question #14, myself being a computer professional with obviously having some kind of specialized skill/training should we select "YES", or this doesn't mean computer related training but something else and hence "NO"??

5) For Question#9, say if I've been in US since 2001 and visited India in 2002,2004,2006 should we mentioned something like as below...
India - 1996 - 2001
USA - 2001 - 2006
India - 2002, 2004, 2006

6) For Question#18, as I'm driving to Vancouver should be select "YES" and for mention "Driving back to USA (my address) on mm/dd/yyyy"

Appreciate your help in answering these questions.

So its better to download one which expires on 09/30/2008

Its not mandatory to fill online, you can download it and fill it up with pen

write "NONE"

You should tick "NO".

India - 1996 - 2001
USA - 2001 - 2006
India - 2002, 2004, 2006

Just write USA - 2001 (NO NEED TO MENTION THE DURATION AND YOUR NATIVE COUNTRY)

Its advised to tick "NO"
