Q9 asks "List all countries you have entered in the last 10 years"

Do I need to include both US and India?

Also in Q 12 they ask for phone numbers and supervisor's name for the past 2 jobs. What if they have already left that company, and I no longer have their contact information?

Q 17 - Do I need to list all my schools from like 7th grade or something? God who knows the address/ phone numbers anymore.

Q 18 Have you made specific travel arrangement?

What do I say there?
 
I included all countries US, India, etc

In my case , one of my last employer was first acquired by another company and then that branch where I used to work was shutdown. All this happened after I left. So I gave phone number of the branch in california where my compnays headquarters was present before it was acquired.

I listed everything after 10th .

Depends on if u have booked any tickets , made any preparation for travel which is on record.

My bank has issued me a "Cashier's Check" for the fee, they say it is same as a bank draft as the money is already withdrawn from my account.

Is this going to be okay?

Also it says that "Void after 90 Days", so what if it takes them more than that to get to my application?

Is this okay?
 
The check will be cashed within 1 week after St. Louis get you application. That will not take 90 days since the day that you got the check.
 
What should I write in Q3, Full name is native alphabet, Q4 Tribal Name and Q8 Name and address of contact person or organization in US?
Help is appreciated.
