I will be sending my family passports for revalidation to St.Louis sometime this week.
I am here in the US since 2001 and am confused as to write what is few columns:
1 Item 14 National Identification Number : Is that Social Security Number? Or N/A since I do not have any Indian Number?
2 Item 15 Home Address:
Is this Indian address or my current US address?
3 Item 22 When do you intend to arrive in the US?
I am already here. What to write here?
4 Item 25 Name and address of person who you will be staying with:
Is that my own name again?
5 I am sending this for me, my wife and 2 small kids. Do I need to send separate DS-156 for the kids also?
If so, how can they sign in item number 41 - Can I leave it blank by me signing for them in item no: 40?
6 Another question is: Do you want to go to school in the US? My kid already going to First grade here.. Should I write yes and give the addres of her school? It might seem ridiculous to see such a question from me. But, I have never done this before and am confused too. So, please help!
