DS 156 Questions

Hi,

My first 3 yrs of my H1 visa are expiring in Feb 2003.
My wife would be going to Mumbai consulate in mid Dec 2002 to get her H4 Visa.
She is going to fill up DS 156.
Can some one suggest sample answers for the following questions in the form.

1) What is purpose of your trip.
2)How long do you intend to stay in the US.
3)Who will pay for your trip.

Is their any other form she needs to fill besides DS 156.

2.Until spouse's visa validity or my visa validity whichever is earlier
3.Myself or Spouse...
(if your spouse demanded exorbitant dowry, this is the time to take revenge!! ))
