DS-156 question

Hello friend,

I am planning to send my visa for revalidation, have a quick oqestion for DS-156 form.

Can I write with pen after printing the form with other details. ie 1) item 22 I would like to mention " I am in USA presently"
2) item 2 I got my new passport here in USA, but not able to get USA when I pring the form in country column !!

If you select the list of countries for question #2 one of the entries is United States of America. Try again.

Actually I was able to select USA from the list, but when I hit the continue, the country field was coming blank for some reason !!but looks like I am getting it now !

But, still I didn't get the answer, whether I can write on the form or not, because in item 30 I want to mentioned that I have been issued US visa twice and there is room for only one date in the form.

--- you CAN write on form, better use black pen
because in item 30 I want to mentioned that I have been issued US visa twice and there is room for only one date in the form.
------ for item 30 you can write "see attached sheet A "and then on SHEET A explain or write and attach the SHEET A, dont forget to sign the sheet with date and your name on it
