DS- 156 Question--Urgent..

DOS insist for Month DD,yyyy format for date of birth. But when I typed it in DS-156 its automatically changing to mm-dd-yyyy format.. Is there anything we can change to make it different format or Can I send it with mm-dd-yyyy format?

Also please let me know whether DOS insist to print DS-156 front and back or 2 sheet printing is fine?

Ignore first error and proceed, I was able to enter 02-04-2003 successfully...
