Given Name: bbb ccc ddd

All my previous visas were stamped as
Surname:aaa
Given Name: ccc bbb ddd

Now I am planning for revalidation in Canada
My 797 approval has
Surname:aaa
Given name: ccc bbb ddd

What should I enter in Given name column of DS-156?

Thank you ginnu. I have one more concern


I scheduled appointment at Ottawa end of this month for H1B renewal. My employer says that I129 was misplaced. Please can you advise if that document is important.

Also do they ask I129 at the consulate window. My education was in India. will that be a problem?

---------------- copy of I-129+LCA that was submited to USCIS, lawyer who filed H1 may have the copy
Also do they ask I129 at the consulate window.
------------Yes, they CAn ask any document like certified copy of I-129+LCA, employer letter
Here is the list that they can ask:
Required Documents for Visa:

*** H1B To H1B ***
1. Original I-797
2. Copy of LCA (Labor Condition Application)
3. Copy of I-129, certified by your lawyer or company
4. Current employment letter from your company Human resources, stating your position with the company, date of employment and salary information.
5. Original degree and transcripts
6. Passport with I-94. Passport must be valid for at least 6 months.
7. W-2 forms
8. Recent check stubs
9. DS156 application form with one passport size photo (Two DS156 and two photos for following countries: Cuba, North Korea, People's Republic of China, Russia and Vietnam.)
10. Appointment letter for visa
11. Federal and State Tax return
12. Information from previous employment



My education was in India. will that be a problem?
