30. Have You Ever Been Issued a U.S. Visa?
WHEN?____________
WHERE?___________
WHAT KIND OF VISA?______________

Is this mean, do I need fill current H1B visa approval details?

Do we need to fill DS 156, DS157 one for each person separately? I am the primary (H1B) and I have two dependants (H4)

Can someone clarify this question please?
