DS-156 & DS-157 Form Questions

DS-156:

I recently renewed my passport from San Franciso, CA. So do I input San Franciso, USA, CA in box 2?

DS-157:

#8. Full Name and Address of Contact Person or Organization in the United States.

Do I list my company's name since I dont have any relatives in USA?

#10. List All Countries That Have Ever Issued You a Passport

Should I list India & USA, since I recently renewed by Passport here in CA.

The issuing country is India so only India. if it asks for the location/name the consulate then mention SFO etc

List companies contact (assuming for H1)

Indian Consulate San Franciso or India

DS-157 is very clear to me now.

However correct me if I'm wrong, but in DS-156 for #2 Place of Issuance, the City / Country / State-Province would be San Franciso / USA / CA and for # 3 Issuing Country would be India.

A yes/no answer would suffice. Thanks again.

------ YES and if confused write on cover letter and sign the cover letter write Item# and explain
