Has Anyone Ever Filed an Immigrant Visa Petition on Your Behalf?

i have applied for my I -140 and wife applied for 485 and 765 along with me.
----- YOU will write YES as I-140 for you is filed if no I-40 filed for your WIFE then she will write NO as I-140 is immigration petition
Both of us have got the EAD Card.
What should I be writing in this question?

What should I write? (When I went for stamping to Mumbai in may02, I wrote my Mumbai address) Should I write my California address now since I am in the US?
25: Who will you be staying with in the US
I have seen previous posts suggesting mentioning spouse. I am not married however.
27: What is the purpose of your trip
30: Have you ever been issued a visa?
I have been issued two visas (F1 and H1B) Should I write first, second or both?

DS-157:9 what are the countries you have entered in the past 1 years?
i had entered Hong Kong for 5 hours while on transit back to india. I have HK immigration stamps in my passport to reflect this.
Should I mention Hong Kong for Q9?

DS-157:Q17 "List All Educational Institutions You Attend or Have Attended. Include Vocational Institutions But Not Elementary Schools."
how far back in time should i go? (bachelors? 11-12? 1st -10 standard?)
also it asks for dates. Do i have to give the EXACT dates or will month/year do?
