Doubts need assistance

Hello

I have been selected for DV2021. I have a few doubts and need assistance.

1. I travelled to the US around May 1995. However i lost my passport which had the visa stamp and travel dates. Now, when filling out the ds260, they are asking for dates when i visited and i can't remember. What can i do?
2. My name on my national ID card has been misspelled. This is the document that is used to issue police clearance certificate which means that the PCC has a slight misspelling on my last name. The process of getting the name corrected on ID is time consuming and may delay my application process. Can i submit a sworn affidavit to show that the 2 names are of the same person?

1. Write an estimate. You can explain in interview.
2. It will be months before you need a PCC. PCCs expire so you don’t want to get one too early. So, you have time to correct it.
 
You have plenty of time to fix the name error on your national ID, so fix that first rather than replicate the error with the mind of addressing it with a sworn affidavit.
