You file form ETA 9089 for PERM which does not require any signature.You can view the form at http://workforcesecurity.doleta.gov/foreign/pdf/9089form.pdf

PERM is online filing so no signature is required but if you get an audit request, as I did, you will need to sign on a certain document to be attached with documentary audit reply

Please note!

If you get audited the signature dating of your's and HR's signature should be the date that you filed, not the date you got audited or anything else. That's why it is best to sign the document printout on the day you file (so you don't mess up). At least that's what my lawyer says is important regarding signing and dating.

my lawyer says there is nothing in the regulations requiring this so it should be fine to date accurately to the present.... I agree with your reasoning though so I guess we'll find out based on my PERM result if there is any issue.
