I was wondering if all i-485 applicants get RFE before approval. Did anyone get approved without RFE? If, only some people get RFE, then is there any rule about who gets it and who doesn't?

All approvals after Aug, 2001 have been thru RFE. I am yet to see one without. There was one case in Oakland which was'nt thru RFE. It seems issuing RFE has become more of a rule than exception now.
