Does anyone know any case that I-140 is denied while EAD and AP are approved?

I know a case Where EAD was approved and I-140 was revoked ( before approval of I-140).

When it was revoked ? After 180 days of 485 or before that?
What r implications of that?
If person is already using EAD what is process of coming back on h1b?
Does person needs to go out of country to get h1 visa again?
I would like to extend thread for people for whom 140 was revoked or reference point for unfortunate people whose 140 will be revoked.
