Does anybody have a letter stating use of AC21 drafted by a lawyer?

AC21 Format
Hi,
This is what my lawyer used when we notified INS about my change of employers:
Letter to INS RE: LIN-XX, Name of applicant, I-485 under AC21
In the body of the letter:
-case background: stated that I started with Company A, with H-1B visa, when Labor certification was approved, what was the job description and the the following duties under that job
-portability: stated that AC21 can be invoked under 2 conditions (state conditions here)
-current situation: That I was working for Company B now, with EAD effective until mm/dd/yy, employed with the same or similar job description with the same pay.
-conclusion: We (with the lawyer) are requesting the INS continue processing I-485 under AC21 law, because it has been 180 days since the filing. And that the Company B intends to employ me in the same job classification for which the labor certification and I-140 was approved.
-thank you, blah, blah, blah
Attachments included copy of:
-application for alien employment certification
-I-140 receipt
-I-485 receipt
-EAD
-employment letter from Company B addressed to the INS, stating that I still work for them.
