Does any NIW case get rejected without RFE?

I would like to know if any NIW case is rejected by INS without RFE?
It seems to me that INS always RFE in every NIW case rather than simply denies it.
Am I right?

Yes, I know two NIW cases that were denied by VSC without issuing RFEs. They issue RFE if the case has at least some merit in it.
