Documents to be send with PIO application (SFO consulate)?

I want to apply for a PIO card for our new born infant. Both my wife & I are on a green card. Are the following documents sufficient? I don't plan to send any original passports (I will send notarized copies only)
Anyone know what's an "apostilled copy of the US passport"?

1. Application form with left/right thumb impression
2. Two photographs (2 inches x 2 inches or 35mm x 35mm photos). One photo pasted and the other stapled
3. Cashier's check for $185
4. USPS Express mail Return envelope
5. Apostilled Color photocopy of the Baby's PassPort (pages with particulars only)
6. Notarized Color Photocopy of brith certificate of Baby
7. Notarized Color photocopy of the first 3 and last 3 pages of Father's and Mother's PassPort
8. Notarized Color photocopy of Driver's Licence (Father & Mother)
9. Notarized Color photocopy of Green card (Father & Mother)
