Documents required for I-140/485

What are the documents that is required for 140/485 filing? I heard that we need to produce certificate for vaccinations (Mumps, Measles, Rubella) that were taken during childhood? Most people don't even bother to retain those certificates. Please advise.

When you are required to produce vaccination certificates, they
do not mean produce the certificates from your childhood.

You need to go to your primary care physician ( check this with
the INS regulations regarding vaccination requirements ) . The
doctor would suggest tests to check your immunity levels
( anti-bodies ) to all the diseases listed.

The laboratory would do the tests and send the report to the
doctor. Based on this report the physician determines your need
for different immunization shots.

Once this is done the physician would give you the Immunization
chart for producing at the INS.

Thanks Raju, but isn't it like if you produce that certificate, you can skip the vaccinations etc.

It is true. If you are able to establish the immunization history,
then the physician would determine what can be skipped.

In the absence of the record, they have to do the tests to
determine the immunizations needed.
