Documents required for H1B after J1 waiver is approved

Hello,

After DOS/USCIS approves the case, what documents need to be submitted with H1B application. Here is what I know:

1) State license
2) LCA, form ETA 9035E

Any other documents, e.g. I94, copies of passports, marraige certificate for spouse, birth certificate for kids (for H4) etc.
