Documents required for AP.

Hi Guys,
I am going to apply AP on my own. Does anyone know the list of the documents to be attached with the AP application. I am thinking send the following with I131.

1. 2 immigration photos
2. 485 receipt (I applied 485 in FEB\'02)
3. Copy of the passport
4. Copy of I-94
5. Copy of H1B
6. Check Payable to INS for $110(is this the correct amount??)

Forgot one more thing:

A letter describing the reason for applying for AP. I wrote, "plans to meet family, friends, etc." and i don\'t think passport copy is reqd.
