Documents needed for filing AP

I just got 485 receipts from my lawyer and want to
file AP by myself. What documents do I need to attach with
I-131 besides copy of 485 receipt and two photos?
Do I need to attach a letter explaining why I need AP?
Do I need to attach my current and previous non-immigration status papers?
Do I need to attach copies of passport including the visas?

Anybody with such experience please help.

Letter explianing why you need AP - YES, others NO. Write your receipt numbers and A# in all the documents you are attaching in bold (back of photograph)
