Documents for OCI and SC Washington DC

Hi,

I am applying OCI for family and SC at the same time. Do I have get SC for my son(minor) also? What are the documents do I need to send for SC?

For OCI I am sending the following documents.

1. Part A + Part B
2. First two and last two pages of US passport.
3. Indian passport first 3 pages and last 3 pages.
4. 2 photos
5. Fees(275$ + 20$)

What documents do I have to send for SC? Please let me know.

You need to have following besides your checklist items:
1. $175 check for passport cancellation each person
2. Proof of address
3. $20 check for postage

Thanks for the reply. Do I have to send Original Indian passports? Do I have to send two seperate $20 checks(one for OCI and another one for SC)?

Thanks for the replies. What about the pictures? Do I have to send 4 passport photos(2 for original and 2 for duplicate)? This is so confusing. Please let me know. Thanks again for any input.
 
Pictures...

Paste 1 51mmX51mm picture in each PART A of the application. Include 2 additional such pictures. So total 4 pics (1 each on the 2 PARTA copies and 2 additional)

Nav100,

Please read (several times) the page on renunication form and OCI form at the Embassy webpage.
I am unable to post the link here since I do not have >15 posts...
It very clearly says 2 copies of Renunciation form. It is going to be tough to get it all right without reading that webpage...

Thanks Satish. Do I have send 2 sets of Indian passport copies and US passport too for Renunciation? So total would be 4(2 sets for OCI and 2 sets for Renunciation).
 
Question on attestation of supporting documents for OCI at Washington DC

Dear Folks, As per Washington DC embassy website's suggestion, I am sending SC and OCI forms together.
In the OCI application, what supporting documents do I need to self attest or notarize
I am keeping the following:
1) Nat. Cert - copy
2) US PP pages - Copy
3) Indian PP pages - Copy
4) Driver's license - Copy
5) Rental Lease agreement - Copy
Also, I am planning to keep two $20 money orders - one for the SC application and one for OCI.. Is one enough??

You need to send ORIGINAL Indian Passports along with the copy in the packet and One $20 MO if you are sending it togeather. If notarization is free, then get all the photo copies notoraized, else self attestation is enough.
 
how many photos - 3/4 or 5?
also what should I write for US passport issue place? Is it USA or US dept of state( there is not enough space for this).
