Documents for EAD and AP, Jaxen, Jim and other pl comment

I am planning to file EAD and AP renewals by my self.

Please check following list and update me if I miss any document.

Documents for EAD
1. 2 passport photos. Write A# and name on the back
2. Form 765
3. Copy of 485 receipt.
4. Copy of last EAD receipt
5. Both sides copy of last EAD card
6. Self Check/Money order/cashier's check $120.
7. Signature card

Documents for AP

1. 2 passport photos. Write A# and name on the back
2. I-131 Form
3. Copy of 485 receipt.
4. Copy of last AP receipt
5. Copy of current AP
6. Copy of I-94(both sides)

I have the following questions.

1. How to get signature card? Can I download from somewhere or should I go to INS office to get one?
2. Should I send EAD and AP in different covers? What is the best service (FedEx, USP, US postal)?
3. Do I need to add any cover letters? If yes, can someone post a sample letter?

signature card is not rquired any more.
You need a letter jusstifying AP (parents in home country, etc)
Cover letter make the package complete. " I am applying for .. ,. My details are ..., please approve"
I have always found US Post office - priority mail with delivery confirmation effective and cheap.

The photos for EAD and AP should be immigration photo, not passport one.

The EAD issue paper shold be attached.

Do you know how many copies of the form I-765 and I-131 for each person?
