Document for H4 visa

All,
Just checked the reqd. documents for H4 visa stamping...

It says pay slips for the current calendar year and federal tax returns (IRS Form 1040 and W-2s) for all the years in which he/she has been employed in the U.S. on the H-1B visa.

Does this mean if I apply for my spouse's H4 in Dec 2003... should i submit pay slip for 1 full year?

Also please let me know if I can submit the copy of my I-797 and LCA or should it be original?

few payslips should be sufficient.
certified copies are accepted.
