Document for H1

Could any one please let me know what all document required for H1- Extension (including Fee for USCIS)

LCA to be approved by DOL, form I-129 + cover letter, past H1 approval copy, copy of valid I-94(front and back) AND recent 2-3 pay stubs, I think USCIS fees is $ 130+ lawyer fees
