Do we get a Name Change certificate?

When someone gets a name change done as part of their N-400 process, do they automatically receive a name change certificate at the Oath ceremony (or later)? Or do they need to apply separately for such a certificate?

I have heard conflicting responses so am curious as to what people have experienced.

I was asked to sign 2 copies of my name change certificate at the interview and was told by the IO that one of them would be returned to me at the oath with the court's seal, along with my naturalization certificate. This is exactly what happened.
 
I had my Oath today, changed my name,
but didn't get a name change certificate...

Do I really need one, or will the Naturalization Certificate
be sufficient to have my name changed with
the different agencies ?

Where can I request one ?
