I entered back in 1992 and about 7 years ago I got married and have two children; until NOW I'm applying for my GC I paid for help for the first papers I received the approval noticed to change my status so I started the process... by myself...

My question is I lost my I-94 back in 1993 do I need to apply for another one in order to submit my papers / form I-485.

Thanks for the help.

since I did'n receive any answers from this site I'll post what I found.

I call the office of my congressman they usually have free immigration services and I was told by a very nice lady that I need to have that document in order to fill out the form I-485 and the one to request worker's permit.

So the form to request a replacement for the I-94 is : I-102 and the fee is $320. (May 2010)

I also found that you can request to wave the fee, and your congressman office can supply you with the form need it for that :o

on what status you entered US?
if your employer filed H1 extension then you may have got the latest I-94 with your H1 extension approval notice.
We need full details of your case from beginning.

I entered in 1992 with a tourist visa.
I just never left.

you are eligible for 245(i)
if you don't have the I-94 and stamp in your passport has entry date attach the passport page that has entry info and submit affidavit that you have lost I-94.
