IMO, the two affidavits along with the birth certificate should suffice; assuming that your BC
contains the names of your parents, you could get an additional affidavit from your parents stating that though the BC does not mention your name, it does pertain to you.
Also, if the affidavits contain a signed self-attestation clause, they need not be notarized.
