I am flying to USA soon to activate my immigrant visa from 2009 DV program. my questions:
1- do I have to fill form I-94 which they distribute in the plane before we land?

2- when someone leaves the US, do they stamp his passport? if not how do they keep track how long one stayed out side the country?

No, this form is for foreign visitors entering the United States with a non-immigrant visa. Once your passport was stamped with I-551, you no longer need it.
