DIY EAD Renewal

It's been 2 years and I haven't seen the light at the end of the tunnel. My dad's not getting better (chronic kidney disease stage 4) and I can't even apply for AP to go back to visit him.

Anyway, I'm jobless but I need to renew my EAD anyway. I'm trying to save $100 by doing it myself (instead of having my attorney does it for me). From the I-765 instruction, looks like I only need to send them :

1. Copy of I-94 front and back (didn't they take your I-94 somewhere along the process? I can't find mine but I might be able to request a copy from my attorney). edit to add: my attorney's assistant just told me they don't need copies of my I-94 for renewal, just copy of EAD. really?
2. Copy of EAD front and back.
3. 2 passport photos and $340 check.

Do I need to send them anything else? I called the 1-800 and spoke with a CSR who's really not helping at all.
