discrepancy in m476 document checklist

I am going through the document checklist in M476 and find some discrepancies in the list: The list says:

If you answer “Yes” to any of Questions 1 through 15 in Part 7 of form N-400, send:
A written explanation on a separate sheet of paper.

If you answer “No” to any of Questions 1 through 5 in Part 8 of form N-400, send:
A written explanation on a separate sheet of paper.

What is going on? There is no Q1-15 in Part7, no Q1-5 in Part8. In my N400, part7 is 'time outside US' and part8 is 'marital history'
Is the M-476 old?
Or is my N400 old?

Now that I am suspicious, what is the fee? 675?

M-476 is outdated. Just follow instructions for N-400. The fee is $675 and the fee includes fingerprinting.
