Yes, I did get the yellow form with mine and my wife\'s FP notice.

Rd-12/12, ND -12/26, Recd. FP notice on 07/03/01. No yellow form. Only appointment letter. How important is the yellow form and what does it contain? It it a problem, if I have not received it.

The yellow form is called "Pending Form I-485 (EB)", and on that form INS requests you to submit your same employment letter that was filed at the time of I-140.INS says that it is not required to reply to the yellow form, but if you do so, you assist INS to make decession on your case. I guess, people usually receive it with there FP notice if there application of I-485 is mature for more than six months.
