CP interview - Documents to be prepared and carried - Let\'s talk

Guys,
 Let us start a discussion on what are the documents we need to carry at the time of the CP interview. Few I know of are:
1. Filled P4 forms
2. Medical documents (to be obtained at the local hospital at India)
3. Employment letter notarized.
4. Current Pay stub.
5. Sailing Permit (not sure)
6. 3 years tax statements.
7. Original degree docs, birth certificate etc.

Let us know what else do we need to carry?

Also heard that we need carry the following too.
 1. Bank deposit/withdrawal consolidated statement for one year (my lawyer asked me to get it from the bank)

Must one:
1. Filled DS 230-I, DS 230-II unsigned, I-134(one for each dependent, original notarized) , IRS9003(Primary Applicant & spouse)
2. Medical documents (to be obtained at the local hospital at India)

5. Birth Certificate/Affidavits.
6. 3 years tax statements.
7. Marriage certificate.
8. 3 months Bank Statement.
9. 3 immigration photos.
10. PCC.
11. Passport.

Additionally you can carry wedding photos, H1 approval, Labor Cert, I 140 approval, Degree cert, previous exp letters, company docs etc, etc..

Help in filling I-134

I am confused a little about I-134 form, I had filled it up long ago once for my parents visit.
Could someone help me In filling it up for CP.( This form is available for download at chennai embassy website).
Item 3 and item 8 are confusing to me. I hear from various sources that I134 needs to notarized for each dependent separately, I have 2 dependents, spouse and a kid.
1. What do I fill up in item-3 and Item-8.?(They both seem to be conflicting, do I got to repeat the name again).
2. If I mention my spouse name in item 3 and also mention the kid\'s name as persons accompanying/joining, doesn\'t it mean that the affidavit is executed for both automatically, then why do we need to execute separately for the kid?
 
I asked this to my lawyer earlier and ihis is what they say..

Item 3: Lawyer agrees it is confusing and asked me to keep "Name of Spouse and children.........." fields blank for all dependents forms.

Item 8: put names of all the dependants on all I-134s.You do need to have seperate I-134 for each dependant.

CP gurus, please confirm.

I am also confused about I-134. I am primary applicant - however my spouse is not my dependent because spouse is working. Do I still have to fill out I-134? please advice.

This is what they say about item 3 of I-134.

You must give details about your derivative spouse and child.

 This is the full list from immihelp web site. Please take a look.

Before leaving US for the CP interview, please have following documents ready for the interview. Remember, more the better. If you have doubt whether particular document may be required or not, just carry it. But don\'t open your bag unless asked for a specific document.
Appointment letter for immigrant visa interview.
DS 230 Part II for each applicant. It is part of Packet 4.
Filled Packet 3 forms that consulate mailed you, if you haven\'t already sent them.
Passport with at least 6 months remaining. Bring all old passport which have US visas.
Birth certificates/affidavits. Original + 2 copies. No need to get the copies notarized. They will take a look at the original and copy. They will take a copy and give you back the original. If no birth certificate, get non-availability certificate, 2 affidavits and also carry the school leaving certificate(You know how valuable school leaving certificate is in INDIA and how it is used.)
Sample Birth Affidavit
Marriage certificate/affidavit, if married. Original + 2 copies. No need to get the copies notarized. They will take a look at the original and copy. They will take a copy and give you back the original.
Degree certificates/transcripts.
Any work experience letters used for labor certificate.
Originals of all the documents used in I-140 (reccommondation letters, articles etc.)
Notarized employment letter. 2 copies.
Company brochures, annual reports.
Company financial statements.
Bank and other assets statements like stocks etc.
W-2s, personal tax returns for last 3 years.
Sailing Permit
Recent pay stubs.
Take any proofs of vaccinations you/dependents have.
Form I-134 Affidavit of Support for the dependents.
Notarize it in US. Sign it in front of Notary. Notarized in India NOT accepted.
Also, if your spouse is employed, it is better to carry an employer letter for him/her too.
IRS 9003 Additional questions for the applicant for permanent residence. This is an IRS form and not INS. Each applicant including dependents should fill this form. You have to write you SSN (or ITIN) and four questions regarding income tax. No signature is required on this form. It is a part of Packet 4.
All previous H1s, F1s, EADs, APs, I-485 receipt etc.
Apartment lease papers.
Social Security cards of each family member who have social security number. For others, ITIN number cards.
Police certificates, if required for you.
If you have any US born (US Citizen) children, you should carry proof of their citizenship.(Birth certificate, passport etc.) The consular officer really wants to make sure that your children are really US citizen, you are not making any mistakes regarding their status, and hence they are not deprived from getting green cards, if they need it.
Marriage photographs, marriage invitation card.

Lprasad: The reply you got seems to be vague.
The advice CPers got seems to be reasonable, either way the best thing to do would be to take additional blank forms and sign it in front of them if necessary.

What about item10: "That I have submitted Visa petitions on behalf of the following persons".
Should we fill up our spouse and kids name?

Guru\'s... Help needed, Please confirm.
 
Hi Guys...Search for SRIFX and you would find a very good

compilation of what needs to be taken etc. There are a couple of minor changes to his list like attach the PCC at the end etc etc.
I used his list to guide me thro everything.
