I have my birth certificate but my last name is misspelled (2 letters are missing)..do i have to take affidavits from my parent for this..if yes can you direct me to the format of affidavits ..also i dont have school leaving certificate with me is that an issue...

Not a major issue

Misspelled names are acceptable to some extent. It would be good to have parents affidavit for "Just in case" situations.

All they have to mention in the affidavit is your Name and DOB,Place of Birth.
Get it notorized

Thanks what about School Leaving certificate do i need it...

Also if u r aware abt the standard format of affidavit on any website
or anyone u know whai may have done it then it will be helpful...

GCWorth: I had the same problem

Actually my name mentioned the BC was very different (not just mis-spelt). I carried an affidavit from parents, myself (saying that I have not used the name mentioned and xyz is my official name, etc) and the school leaving certificate. The consulate dude took only BC and the school leaving certificate, which had the correct name. My guess is they prefer school leaving certificate to affidavits, as it is more official. Any way u can get a duplicate of yours ?

Class X or Class XII certificate also have your DOB on them

Either one is fine.
It is not a must but "Nice to have" for your own comfort. and they are relatively easy to get.

Rajesh Paul

Rajesh,
My parents had named me ABC at the time of birth, but in school and college I have used XYZ as my name. Is an affidavit from my parents indicating that ABC and XYZ are one and the same person enough. PLease let me know. DO I need to take any more documents in that regard.

I had the same problem

I took an affidavit from parents and one by myself (saying I had not used the earlier name and these are both the same person). The consulate didn\'t ask for either. Instead, they asked for school leaving cert.

Date of Place

Friends,

In PassPort and Schools\' certificate my wife\'s birth place is recorded as XYZ place but in her birth certificate (which we obtain recently) it is mentioned as ABC.
But we know that ABC is a local area inside XYZ.
Could any body confirm what I need to do?
Will it be an issue in CP?
I will appreciate your inputs.

Thanks

take some proof that ABC is a subdivision of XYZ

Does not sound it will be a big issue. Just to be safe take proof

If XYZ is same place as your consulate then it is reasonable for them to know that ABC is a subdivision. But if it is a different city then you might wanna take some thing to proove that.

At the least take affidavit from some one infleuncial from city XYZ saying ABC is a subdivision of XYZ.

Thanks Patta Hara But...

Thanks for your input Patta Hara.
Could you please tell me whether affidavit has to be from some official person or her parent\'s affidavit would be ok.
Thanks again.

Native of city XYZ

Need not be parent to execute the affidavit.. on the second thought, it might be better if that person does not have the same last name.

What you wanna say in the affidavit is that I have lived in city XYZ for and confirm that ABC is a Subdivision of XYZ.

I dont think you will need it...but would be good to have in your back pocket "just in case"
