Just want to confirm courier delivery address:

For check placement, instructions say: Place all the checks on the top application. So should all three checks (I-130, I-485, and I-765) go in one place? On the top of I-485?

Usually when I send a FedEx, they want the receiver's fax number. What phone number should I put for the Chicago Lock Box facility?

You got me there - I don't know what fax/ph no. you can give for Chicago Lockbox. I used USPS Priority Mail service - second day delivery with tracking option. Worked for me!
