Couple of questions for completing the application

I recently got married to a USC and have couple questions:


1) I 130 Question 22
Your relative is in the United states and will apply for Adjustment of Status to that of a lawful permanent resident at the USCIS office in:

Which location should I fill in here? I thought all applications go to the Chicago Lockbox? Where do I get list of offices and do I just pick one that is in my state?

2) Form 765 Question 11:
Have you ever applied for Employment Authorization?

I am currently on H1 and also my Labor petition has been approved by my company. Should I answer yes to this question. If yes then which USCIS office should I mention - the H1 or the Labour petition office or should this be No since I have not applied for EAD before.

3) Can I use form 864 EZ with the filing of form 485 since all three conditions are true or do I only have to use 864.

4) Do we need to have a name change done for my wife (surname) before the application is filed.

Answer to 4: Surname (Last Name) doesnt need to be changed. There are many women who do not change their last name after marriage. My wife keeps her last name as it was before marriage.
 
1) enter your zipcode here to find the field office closest to you.

3) If all conditions for 864EZ are met, use that one. It's much shorter and less hassle than regular I864 (hence E-Z)

Q2 - Answer should be NO if you haven't applied for EAD ever before. Approval of labor petition is not considered application for employment authorization. Just say No to that question.
