they sent me by email attaCHMENT TODAY stating that my sponsor has to fill that. this application has infor : like starting date and ending date, why physician leaving, what did physician achieved on HPSA site, how many poor patients without insurANCE are see,what is going to happen to clinic once physician leaves, did he finish his obligation etc.,
did anyone got this kind of survey?
please, experts guide me again!(YOU HAVE NO IDEA HOW MUCH THIS FORUM HAS HELPED ME AND MADE ME WISER)

Donot worry abt that at all just give that form to ur employer and u will have to sign in the end it is just a proof that u r actually serving the underserved U will have to fill thsi form every year :)
