concerning affidavit please help I am confused

Hi guys , I have been selected on dv 2006. my interview is scheduled in december.
But I have a problem with the Affidavit of support needed. what form should the sponsor use . I-134?
what are the documents needed to confirm the statements of the affidavit.
since on the I 134 bank statement and job statement are only needed. but I have in other forums were needed but they dont appear on the form.
Please any input will be more than helpful. thansk

I-134 and whatever is listed in it. In some rare cases consuls ask to also bring it W-2 and 1040 forms at the interview.
