Checklist Form!

I have received the 2NL. In one of the pages it says:
We request that all items listed are submitted in the following order:
1. checklist for immigrant visa application (please ensure that the address and phone number that you list on this form is complete and accurate. we will contact you through this address)
2. two photographs...
....

However, there is no checklist form included in the envelope!

Does anyone know where to obtain the checklist form?
Should I contact the consulate or KCC for this?

Mostly the 2NL is a standard letter and many had things on there that didn't address their case and just ignored these things.

Bring all your originals and copies with you to the interview and proof of funding or job offer and/or I-134 and you should be fine.:)
