Check the latest I-765 form available on uscis.gov

I was under same dilema. But then I spoke to my attorney they said it was the same thing. BUT I still went ahead and sent in favour of US dept of homeland as thats what the latest form said.
It was just last week so I dont know if it was right or wrong But Iam pretty sure it was . I will keep you updated once the checks hit my bank

Usually, I was told by my lawyer to write "Department of Homeland Security".
I don't know if US before it is needed or not. Both of my EAD and AP were written that way and was approved.
